The only time I've used miles to upgrade has been for long-haul international flights. For me, the extra space and relative quiet is worth it for a 10-11 hour flight. Domestically, I have never used miles to upgrade. Domestic first-class in the US is not worth it, when I can save those miles for a free trip.

Nope, domestic first class it not worth it. The free tickets are nicer.
